You can only change your health plan during certain times, including:

  • In the first 90 days you are enrolled in a Managed Care Plan
  • During your annual open enrollment period
  • If you meet one of the State-approved "just cause" reasons to change plans

  • Talk to a Choice Counselor Monday-Friday, 7am-6pm CT, to request to change your health plan or request a change Just Cause Transfer.
  • Use the automated health plan change system any time to change in your first 90 days or during Open Enrollment. Listen to the message then follow the prompt to make your change.

By Mail

You can request to change your health plan by mail by filling out a Health Plan Change Form. A copy of this form is included in the welcome packet you received. Mail the change form back in the return envelope to:
